PII's pyramid is divided into four parts: human performance, equipment failures, organizational and programmatic (O&P) issues, and management issues. Each part represents a group of symptoms that transcend all problems. PII uses these four parts to represent the foundational basis of every aspect of our technology and expertise. Our services usually combine one or more, and sometimes all, of the pyramid's four parts.

In general, problems typically present as equipment failures or human errors, sometimes involving injuries. However, the problems are most likely the result of O&P problems. When O&P problems are severe or prevalent, management problems are most likely the cause.
