Our Foundation
PII was founded in 1987 by Dr. Chong Chiu (Ph.D., MIT, 1977, ANS Fellow) with the goal of improving safety and performance through root cause analysis, management consulting and training, initially in the standards-intensive nuclear power industry. During the proceeding decades, PII has conducted over 740 man-years of field research and well over 5,000 root cause analyses and case studies. The result of this exhaustive research is a unique, exclusive technology, which can address equipment failures, human performance issues and organizational / programmatic / management inefficiencies in virtually any industry.
